Position Summary

Under the direction of a Registered Nurse (RN), the Licensed Practical Nurse delivers efficient and effective nursing care while achieving positive clinical outcomes and patient and family satisfaction. The LPN operates within the scope of practice defined by the State Nurse Practice Act, contributes to nursing assessments and care planning, provides direct patient care, and supervises care provided by unlicensed staff.

Responsibilities

• Collect, report, and document objective and subjective patient data; observe and report changes in condition to the RN.
• Contribute to individualized patient goals and interventions; implement and help evaluate the plan of care.
• Administer medications and perform treatments per physician orders; document accurately and thoroughly.
• Communicate pertinent data to the RN and physician; seek RN guidance as necessary.
• Support discharge planning, including patient and family education.
• Assign, monitor, and follow up on duties performed by unlicensed staff, per the State Nurse Practice Act and policy.
• Perform drug counts at shift change; assist the physician during rounds and examinations as needed.
• Participate in shift-to-shift communication; assist with orientation of newly hired staff.
• Maintain confidentiality and protect Protected Health Information (PHI) at all times.

Requirements

• Graduate of an approved practical or vocational nursing program.
• Current LPN or LVN license from the State Board of Nursing.
• Experience in rehabilitative or geriatric nursing preferred.
• Current BLS/CPR certification as required by setting and state.
• Ability to read, write, speak, and understand English; TST verification as required by state law.
